The Client referral process is simple and can be made by phone or confidential fax, a doctor ACAT referral is not required.
Referrals from Medical Practitioner or Hospital Discharge are attended to immediately and our Care Coordinator will visit the client in their home or hospital and assess their care and support needs.
Our Care Coordinators visit potential clients initially at no cost in their home:
discuss client needs and those of the primary carer,
arrange services and appropriate support to meet needs with the clients input and consent
Our Care Coordinators (RN) take care to thoroughly assess clients' needs before establishing a personalised Care Plan to meet their needs and wants.
